You will be contacted by the office to coordinate a time for the following items that will be needed to complete the registration process:

• Proof of Birth (Birth Certificate or Similar Document)
• Proof of Identity
• Medical Records (Immunization records, Physical, Other)
• Proof of Residency
• Custody/Legal Documents (if applicable)
